  • If you are not the current property owner, submit a “current property owner’s authorization” form and one of the supporting documents listed below. The request cannot be processed if you are not authorized.
  • If you are the current property owner, the "current property owners' authorization" form is not required but proof of ownership is required.
  • Note: If ownership is under a company name, a recent certificate of incorporation is required.
