I/we have read and understood the City of Guelph Business Licensing By-law (2009)-18855, and the schedule of that by-law pertaining to the classification of business licence for which I/We am now making application for inspection.
Per Section 3(e) of the By-law I/we declare that all fines against the applicant pursuant to the By-Law or its predecessors have either been paid in full or are the subject of a court approved payment schedule.
The proposed short-term rental(s) will comply with all applicable laws, regulations, and by-laws, including the Zoning By-law, the Property Standards By-law, the Fire Protection and Prevention Act, and The Building Code Act.
I/we shall fully indemnify and save harmless The Corporation of the City of Guelph, its elected officials, officers, employees, agents or others that that The Corporation of the City of Guelph is responsible for from any kind of liability, suit, claim, demand, fine, action, loss, damage, legal cost and/or disbursement, or proceeding of any kind whatsoever whether by act or omission or alleged act or omission for which The Corporation of the City of Guelph may become liable or suffer in connection with the enjoyment of the rights granted by The Corporation of the City of Guelph in connection with the short term rental. I/We agree to provide The Corporation of the City of Guelph of any notice of any liability, suit, claim, demand, fine, action, loss, damage within five (5) days.

It is an offence under section 25(e) to provide false or misleading information on this application or at any time hereafter to any person having authority for the enforcement or administration of the by-law and that the provision of false or misleading information may result in prosecution and/or penalties as set out in the by-law, or the refusal, suspension or revocation of the business licence.

The submission of an application does not entitle the applicant to carry on business pursuant to the bylaw. The applicant is only entitled to do so once a current and valid licence has been issued.

Any licence or application issued under Bylaw (2009)-18555 may be refused, revoked or suspended if the applicant or Licensee is convicted of an offence or has past conviction(s), under any provision of by-law (2009)-18555 or any predecessor of such by-law, or under any other by-law, statute or regulation relating to the business, or otherwise in accordance with the by-law.

Information collected in this application will be used to administer the Business Licence By-law and any other by-laws deemed necessary including but not limited to the Zoning, Building, Fire Prevention, Property Standards, and Sign By-laws. Information may be shared with Wellington-Dufferin-Guelph Public Health to facilitate any inspections required to issue a business licence. In addition, the information will be used for the collection of Municipal Accommodation Tax.
Collection of personal information
Personal information in this application is collected under the authority of the Municipal Act, 2002. The information will be used for the purposes of issuance and administration of business licenses, enforcement of relevant by-laws, and collection of Municipal Accommodation Tax.

Pursuant to section 2(2.1) and (2.2) of the Municipal Freedom of Information and Protection of Privacy Act, information that identifies an individual in a professional, business, or official capacity is considered business identity information. Business information may be made publicly available.
